Bug Fixes for Elements

Fixed bug in BMXYZ element when the field length is greater than the insertion length.
Fixed a bug in loss logging (run_setup command losses file) that resulted in logging of γ instead of βγ in the p column for some loss mechanisms. This bug was reported by forum user galnander.
